What is facial nanofat grafting?

Facial nanofat grafting improves skin quality by increasing elasticity and radiance while smoothing superficial fine lines and stimulating the skin’s own collagen production. Unlike traditional facial fat grafting, nanofat is delivered to the superficial layers of the skin with the goal of skin regeneration rather than adding significant facial volume. If needed, the procedure can be combined with fat grafting to the deeper facial tissues. Nanofat treatment provides a natural and long-lasting result, and recovery is typically moderate.

Nanofat is fat that has been processed into a very fine form. During the emulsification process, the remaining fat becomes highly fluid and is rich in regenerative cells and proteins, including stem cells and growth factors. These are essential for tissue repair and improving overall skin quality.

Nanofat is delivered into different layers of the skin using a cannula and microneedling. The treatment can be combined with Morpheus8 radiofrequency microneedling, enhancing the results through radiofrequency energy, which also helps to tighten the skin and reduce unwanted fat deposits.

Who is facial nanofat grafting combined with microneedling suitable for?

Facial nanofat grafting can be used to treat age-related changes in the skin and to improve overall skin quality in younger individuals as well, for example by addressing enlarged pores and dullness. Nanofat grafting enhances skin radiance, improves elasticity, and reduces fine superficial lines.

If there is age-related skin laxity and/or localized fat deposits in the face, nanofat grafting can be combined with Morpheus8 radiofrequency microneedling. This combination allows for simultaneous skin tightening and reduction of unwanted fat deposits, for example along the jawline.

Nanofat grafting provides a subtle increase in facial volume. However, if a more significant volumizing effect is desired, the procedure can be combined with traditional fat grafting, which can also target deeper tissues to restore volume more substantially.

How should I prepare for the nanofat grafting procedure?

Weeks before the procedure:

  • During your consultation, our surgeon will assess your overall health and suitability for the procedure. Discuss your wishes and expectations openly with the surgeon. You may be asked to undergo certain laboratory tests or other examinations to ensure you are fit for the procedure. Inform your surgeon about any medications and dietary supplements you are taking. Certain medications (e.g. blood thinners, aspirin, ibuprofen) and some supplements (e.g. omega-3 products) may increase the risk of bleeding and should be discontinued according to your surgeon’s instructions.
  • Smoking impairs circulation, slows wound healing, and increases the risk of complications. We recommend stopping smoking at least four weeks before the procedure and continuing to avoid smoking during recovery.
  • Maintain a balanced, healthy diet and exercise regularly. Good general health promotes smoother recovery.

On the day of the procedure:

  • Avoid alcohol for at least 24 hours prior to the procedure, as it may increase the risk of bleeding.
  • Wear loose, comfortable clothing that is easy to put on and take off.
  • Take any morning medications as instructed by your surgeon.
  • If you are given sedative medication, you will not be able to drive yourself home. Please arrange transportation in advance.
How does the NANOFAT GRAFTING procedure proceed?

You will be given the exact time of your procedure during the consultation. Please arrive on time. A nurse will welcome you and review the procedure with you. If necessary, you will be given a mild sedative and pain medication, and the treatment area will be prepared, including local anesthesia.

Fat is usually harvested under local anesthesia from the thighs or lower abdomen and then processed into fine nanofat. The nanofat is transferred individually into the facial fat layer using a cannula and into the superficial skin layers using microneedling techniques. When combined with Morpheus8 treatment, microneedling is performed with the Morpheus8 device. The device settings are individually adjusted depending on whether the goal is skin tightening or reduction of localized fat deposits. As an additional step, nanofat can also be injected with a needle into superficial fine lines.

Swelling typically subsides within 1–2 weeks and is usually most pronounced during the first few days. The final result is always individual. Results become visible at approximately 3 months, with continued gradual improvement for up to 12 months.

What are the aftercare instructions?

Recovery after the procedure is generally moderate and usually takes only a few days. Light work can typically be resumed as early as the day after treatment, if needed. Mild discomfort may occur, and you may take pain relief medication if necessary, according to your surgeon’s instructions.

Keep the treated area clean by washing your hands carefully before touching it and by changing to a clean pillowcase. You may usually shower after 24 hours. Avoid sauna, swimming, and strenuous exercise until the wounds have healed, approximately 1–2 weeks. The small incisions at the fat harvesting site are closed with dissolvable sutures, and adhesive wound tapes are placed over the cannula entry points on the face. These tapes should be kept in place according to your doctor’s instructions, typically for about 1–2 weeks.

It is important to avoid pressure and cold exposure to the face for approximately 2–4 weeks. Cold compresses cannot be used after fat grafting, as adequate blood supply to the transferred fat must be ensured. In the days following treatment, the skin is often swollen and red, may feel slightly rough, and may require more hydration than usual. Mild swelling typically lasts 1–2 weeks. If bruising occurs, it usually resolves within the same timeframe. Most patients return to normal daily activities within a few days.

To reduce swelling and discomfort at the fat harvesting site, wearing a compression garment for approximately two weeks after the procedure is recommended.
